Alsip Little League Tournament Player Verification

In order for your child to be eligible to participate in the Little League International Tournament, all required documentation must be submitted to your team manager before the league deadline.

📋 Required Documents

1. Medical Release Form
   Required for every player.

2. Proof of Age
   A clear copy of the player’s government-issued birth certificate is required. Originals are not necessary unless specifically requested by the league or district.
3. Residency Eligibility
   Families must provide one of the following:
   • Three documents that prove residency — each from a different category (e.g., driver's license, utility bill, voter registration).
     All documents must be dated between February 1, 2024 and February 1, 2025.
   • OR a completed School Enrollment Form signed by a school administrator confirming enrollment as of October 1 of the 2024–2025 school year.

4. Tournament Player Verification Form
   This form is completed by the league using the documents you submit.

💻 Submitting Documents Online

You may also log into your SportsConnect account and upload the required documents to your child’s profile. Be sure all uploads are clear, legible, and submitted before the deadline.

⚠️ Important: Players without completed and verified documentation will not be eligible to participate in the tournament.
