Registration Forms

Below are common forms and documents used during the registration process. Some forms are completed online through SportsConnect. Others may need to be printed, signed, and turned in to your coach or a league official.

Player and Parent Forms

• Little League Medical Release Form - required before a player participates in practices or games.
• Parent Code of Conduct (template) - Alsip Little League may provide a local version of this document for families to review and sign.
• Player Code of Conduct - local league expectations for player behavior and sportsmanship. This form will be provided by the league or your coach.
• Concussion Information Sheet - information about concussion recognition and safety. A copy may be provided electronically or in print.

Residency and Eligibility Forms

• School Enrollment Form - for players who qualify based on the school they attend.
• Residency and School Attendance Eligibility Checklist - helps families gather documents that may be needed for All Stars.
